Permission to Release Conduct Records

Under the terms of the Family Educational Rights and Privacy Act (FERPA), a student’s educational record is, with certain exceptions, held confidential by Colorado State University. Information regarding a student’s disciplinary record is available to University officials with a legitimate educational interest. Disclosure of disciplinary records to anyone else generally requires a written release from the student, except in the case of a court order or legal process, or as otherwise permitted or allowed under FERPA at the discretion of the University.

Please note that records must already exist within the Student Resolution Center in order for us to release them. Students cannot provide advance authorization for the release of records that do not yet exist.

Only the student can complete and submit this form. Forms submitted by anyone other than the student will be considered fraudulent and will not be accepted.
